Every four years, billions of people around the globe tune in to watch the World Cup. For 90 minutes at a time, fans sit on the edge of their seats, gripping their armrests, cheering, and groaning in unison. While it looks like a passive activity from the outside, your body tells a completely different story.

As an internal medicine practice, we often talk about the health impacts of diet, exercise, and sleep. But major emotional events—like watching your favorite soccer team fight for survival on the world stage—can trigger a massive physical response. When you watch the World Cup, your body experiences a literal biochemical and hormonal rollercoaster.

The Chemistry of Victory and Defeat

Your endocrine system, which manages your body’s hormones, reacts to a soccer match based entirely on the scoreboard. Scientists refer to this as “vicarious victory” or “vicarious defeat.” Your brain cannot fully distinguish between you playing on the pitch and you watching the game from your couch.

When your team scores or wins a crucial match, your brain floods your system with dopamine, the “feel-good” chemical. Simultaneously, testosterone levels spike. This surge happens in both male and female fans, leading to feelings of high energy, confidence, and collective euphoria.

However, a devastating loss triggers the exact opposite reaction. Testosterone levels plummet, and the adrenal glands release a massive wave of cortisol, the primary stress hormone. Cortisol prepares your body for a “fight or flight” scenario. When you are sitting stationary on a couch, that extra cortisol has nowhere to go. It lingers in your system, which can leave you feeling physically exhausted, irritable, and emotionally drained for hours—or even days—after the final whistle.

The Cortisol Danger Zone

While a temporary stress spike is normal, the intense cortisol release during a high-stakes tournament can pose genuine health risks, particularly for dedicated superfans. Researchers at the University of Oxford studied fans during a previous World Cup tournament. They discovered that highly devoted fans experienced such extreme, prolonged cortisol spikes during a loss that it put them at risk for chronic stress complications, including elevated blood pressure and compromised immune function.

For patients who already manage underlying health conditions, this hormonal surge is more than just an emotional nuisance. Chronic cortisol elevation can disrupt sleep patterns, alter blood sugar regulation, and increase systemic inflammation.

The Cardiovascular Strain

The most dramatic link between soccer and health is its impact on the heart. During critical matches, the risk of a cardiovascular emergency can change significantly for passionate fans.

The intense stress of a penalty shootout triggers a massive surge of adrenaline. This causes blood vessels to constrict, blood pressure to spike, and the heart rate to race. A famous study published in The New England Journal of Medicine looked at fans during the 2006 World Cup in Germany. Researchers found that on days when the German national team played, cardiovascular emergencies in the host region more than doubled for men and nearly doubled for women.

Whole-Body Health on Match Day

As a DO, my goal is to help patients understand how the mind and body connect. The emotional stress of the game often triggers a chain reaction of poor physical habits. During a match, fans are highly likely to consume ultra-processed foods, high-sodium snacks, and alcohol.

When you mix high cortisol and adrenaline with a sudden influx of alcohol and fried foods, you create a temporary metabolic gridlock. Your blood pressure rises, your liver works overtime to process the alcohol, and your digestion slows down due to the stress response.

How to Protect Your Health This Tournament

You do not have to skip the tournament to protect your health. Instead, you can use a few simple strategies to keep your hormones and stress levels in check:

Move During Halftime: When your body is pumping out adrenaline and cortisol, it wants to move. Use the 15-minute halftime break to stretch, walk around the block, or do a few light exercises to burn off the stress hormones naturally.

Hydrate Strategically: Match-day environments make it easy to overconsume alcohol or sugary sodas. Match every alcoholic or caffeinated drink with a full glass of water to keep your blood pressure stable and support your kidneys.

Practice Intentional Breathing: If the game goes into extra time or a tense penalty shootout, take three slow, deep belly breaths. Deep breathing activates the parasympathetic nervous system, acting as a natural brake against a runaway adrenaline spike.

Fandom is a beautiful way to connect with community, share passion, and experience collective joy. By staying mindful of how the game impacts your physical body, you can safely enjoy every goal, save, and victory.

For years, millions of older adults have reached for over-the-counter glucosamine supplements to ease knee and hip stiffness. Because it is widely available without a prescription, it is easy to assume that it carries zero risk.

However, a groundbreaking new medical study has turned that assumption upside down, revealing a hidden risk to brain health. This critical update reminds us that even long-trusted supplements can hold unexpected dangers.

The New Clinical Data on Glucosamine

The recent warning stems from a landmark study published by University of Florida Health researchers in the journal Nature Metabolism. By utilizing advanced artificial intelligence to analyze 12 years of electronic health records across more than 60,000 patients, researchers uncovered a deeply concerning link between glucosamine and accelerated cognitive decline.

The study revealed two major findings for vulnerable populations:

Accelerated Alzheimer’s Risk: Patients already experiencing Mild Cognitive Impairment (MCI) who regularly took glucosamine faced a 25% higher risk of progressing to full-blown Alzheimer’s disease.

Increased Dementia Mortality: For individuals already diagnosed with dementia, regular glucosamine use was associated with a 25% higher mortality risk within five years.

Why Does a Joint Pill Affect Your Memory?

To understand why a supplement meant for your knees can impact your brain, we have to look at the biology.

Glucosamine is an amino sugar. When you swallow it, it travels through your bloodstream and easily crosses the blood-brain barrier. In a perfectly healthy brain, this does not appear to cause issues.

However, in an aging brain that is already showing early signs of cognitive fragility, glucosamine acts like fuel on a hidden fire. It triggers a hyperactive process called hyperglycosylation—essentially placing abnormal “sugar tags” on critical brain proteins. This abnormal tagging causes the proteins to malfunction, accelerating neurodegeneration and worsening memory loss.

The Demographics Dilemma

This discovery presents a unique challenge for our community. The demographic of adults over the age of 65 is the exact population most likely to take glucosamine for osteoarthritis and joint health. Coincidentally, this is also the exact population at the highest statistical risk for developing early-stage cognitive changes.

What makes this a critical cautionary tale is that many patients take glucosamine “just in case,” despite years of clinical clinical trials showing that its actual ability to rebuild joint cartilage or provide significant pain relief is historically weak and inconclusive. In medical terms, the potential risk of this supplement may now heavily outweigh its proven rewards.

Actionable Advice for Our Patients

If you or a loved one are currently taking a glucosamine supplement, there is no need to panic. The study specifically noted that the accelerated decline occurred in brains already experiencing early cognitive changes, rather than healthy, younger brains.

However, this news is an important reminder to review what we put into our bodies. We advise taking the following steps:

Evaluate the True Benefit: Ask yourself honestly if the supplement is actually reducing your joint pain. If you aren’t sure, or if you are taking it purely as a preventive measure, it may be time to stop.

Consider Safer Alternatives: If you need joint relief, guide your focus toward lifestyle modifications like low-impact exercise and physical therapy, or discuss alternative anti-inflammatory options with us that do not cross the blood-brain barrier.

Bring Your Bottle to Your Next Visit: Bring your complete list of vitamins and supplements to your next appointment at East County Internal Medicine.

Our goal is always to treat the whole person. We want to ensure that the choices you make to protect your joints today do not inadvertently compromise your cognitive health and memory tomorrow.

Have questions about your current supplement regimen? Schedule an appointment with us at eastcountyim.com to review your medications and build a safe, effective plan for healthy aging.

For decades, the medical community accepted a grim compromise in the treatment of severe obesity. To save a patient from…

For decades, the medical community accepted a grim compromise in the treatment of severe obesity. To save a patient from the long-term ravages of metabolic syndrome, cardiovascular disease, and type 2 diabetes, we had to subject them to the acute, life-altering risks of major abdominal surgery. As a physician, I have watched this landscape closely, and the conclusion is now inescapable: the era of bariatric surgery is over.

With the unprecedented success, high efficacy, and vastly superior safety profile of modern GLP-1 and GIP/GLP-1 receptor agonists, weight loss surgery has become a costly, dangerous, and obsolete relic of the past.

The Illusion of a Permanent Fix: Complications and Recidivism

Proponents of bariatric surgery have long argued that altering a patient’s anatomy provides a permanent solution. The clinical reality, as detailed in extensive medical literature and databases like UpToDate, tells a much darker story of early morbidity, late-stage complications, and high rates of failure:

Acute Surgical Dangers: Unlike non-invasive pharmacotherapy, bariatric procedures carry immediate, life-threatening perioperative risks. Anastomotic leaks, staple-line failures, internal hemorrhaging, deep vein thrombosis (DVT), and pulmonary embolisms are inherent to the operating room.

Lifelong Post-Operative Chronic Illness: Surgery does not cure metabolic disease without introducing new, chronic pathologies. Patients are frequently left to manage permanent anatomical damage resulting in severe dumping syndrome, marginal ulcers, chronic bowel obstructions, and strictures.

Severe Malabsorption: By destroying or bypassing the natural gastrointestinal tract, surgery forces patients into a lifelong battle with profound nutritional deficiencies, requiring aggressive supplementation of vitamin B12, iron, calcium, and vitamin D just to avoid neurological and bone damage.

The Myth of Permanent Weight Loss: Despite undergoing irreversible anatomical mutilation, a staggering number of patients suffer from weight recidivism. Over a 5-to-10-year timeline, metabolic adaptation and pouch stretching cause patients to regain the weight they sacrificed their anatomy to lose. Resolving this typically requires highly complex, even more dangerous revision surgeries.

The GLP-1 Revolution: Superior Safety and Complete Autonomy

The rise of targeted medical weight management has completely shifted the risk-benefit analysis. GLP-1 medications allow us to achieve profound, systemic metabolic resets without a single incision.

A Gradual, Manageable Safety Profile: The side effects of GLP-1s are primarily gastrointestinal (nausea, delayed gastric emptying) and are highly manageable through clinical titration. They carry virtually zero risk of sudden post-operative mortality or structural tissue failure.

Preservation of Anatomy and Autonomy: Medical management leaves the patient’s digestive system completely intact. If a patient plateaus or experiences intolerance, the medication can be safely discontinued or switched to next-generation multi-agonists. There is no permanent, irreversible damage.

The Policy Imperative: Defunding Medicare for Weight Loss Surgery

Because we now possess a non-invasive pharmaceutical alternative that matches or exceeds the metabolic benefits of the operating room without the catastrophic downside, healthcare policy must evolve.

It is time for the Centers for Medicare & Medicaid Services (CMS) to re-evaluate National Coverage Determination 100.1. Taxpayer dollars through Medicare should no longer subsidize dangerous, irreversible, and highly volatile surgical procedures when a vastly safer, highly effective medical alternative exists. Medicare funding must shift away from the scalpel and entirely toward expanding access to long-term GLP-1 therapies, optimizing patient safety, and reducing the immense financial burden of post-surgical complication management on our healthcare system.

A New Standard of Care at East County Interventional Medicine

At East County Interventional Medicine, our commitment is to the oath of first, do no harm. We believe that subjecting patients to the structural trauma and long-term failures of bariatric surgery is no longer medically justifiable. The future of metabolic health is medical, non-invasive, and patient-controlled. We are proud to lead the transition into this safer, evidence-based era of obesity medicine. [1]

You may have seen the headlines recently about a major shift in Florida’s public health policy. As of late 2025, Florida became the second state in the country to effectively phase out community water fluoridation.

This decision has sparked a high-profile “face-off” between state officials and national public health agencies like the Centers for Disease Control and Prevention (CDC).

As your trusted dental care team, we want to look past the political noise, explain exactly what is happening, and—most importantly—help you protect your family’s smiles.

🔍 What Happened to Florida’s Water?

Through the passage of the Florida Farm Bill, the state prohibited local water utilities from introducing additives that aren’t strictly required to meet basic drinking water safety standards.

Because fluoride is added to prevent cavities rather than to purify the water itself, utilities across the state—including right here in our local communities—have had to shut down their fluoridation systems.

⚔️ The Face-Off: Florida vs. The CDC

The debate centers on two completely different views of public health:

The State of Florida’s Position: Backed by State Surgeon General Dr. Joseph Ladapo, state officials advocated for the change by labeling mass fluoridation as “forced medication.” They pointed to select studies suggesting potential health risks from high levels of fluoride exposure, arguing that individuals should choose their own vitamins and minerals.

The CDC & Dental Community’s Position: The CDC, the American Dental Association (ADA), and our team view community fluoridation as one of the top public health achievements of the 20th century. Decades of data show that keeping a low, safe level of fluoride in tap water reduces tooth decay by roughly 25%. Experts note that the risks cited by the state involved fluoride concentrations far higher than what Americans ever receive from a standard tap.

🦷 What This Means for Your Teeth

Fluoride acts like a shield for your teeth. Every day, minerals are lost from a tooth’s enamel layer when acids from food and bacteria attack it. Fluoride promotes “remineralization,” putting those vital minerals back and making the tooth surface more resistant to future acid attacks.

Without fluoride in our local tap water, the natural shield is weakened. This means both children and adults in Florida are now at a higher risk for developing cavities.

🛡️ 4 Ways to Protect Your Smile Today

You do not need to panic, but you do need to be proactive. Because you can no longer rely on your tap water for automatic protection, we recommend taking these steps:

Switch to Fluoride Toothpaste: Ensure every member of your family is brushing twice a day with a toothpaste containing fluoride.

Incorporate an Anti-Cavity Mouthwash: An over-the-counter fluoride rinse adds an extra layer of defense after brushing, especially for children prone to cavities.

Ask Us About In-Office Varnish: During your routine cleanings, we can apply a professional-strength fluoride varnish directly to your teeth. It takes seconds and provides months of targeted protection.

Consider Supplements if Necessary: For young children whose permanent teeth are still forming under the gums, dietary fluoride drops or tablets can be prescribed if they are not getting it from other sources.

👋 We Are Here to Help!

Public health policies may change, but our commitment to your family’s health never will. If you are concerned about how these water changes will affect your dental health, let’s talk about it at your next visit. We can evaluate your specific cavity risk and build a personalized protection plan just for you.

The obsession with Vitamin D has officially crashed into reality. For years, daytime television medical gurus and wellness influencers bombarded us with claims that the “sunshine vitamin” was a miracle cure-all. We were told that boosting our blood levels could magically fix fatigue, cure low moods, and prevent chronic diseases.

At East County Urgent Care and Immunizations (eastcountyim.com), we advocate for evidence-based medicine, not media-driven trends. The truth is out: the multi-million-dollar wellness hype machine has misled millions of healthy people into ordering unnecessary lab tests and consuming massive, unmonitored doses of supplements.

The financial and clinical consequences of this overtesting trend have forced federal action, and it is time for patients to stop the unmonitored pill-popping and focus on basic, healthy nutrition.

The “TV Doctor” Hype Machine

The obsession with Vitamin D did not happen by accident. It was heavily popularized by celebrity doctors who took minor, observational studies and blew them completely out of proportion. These media personalities framed the average person’s normal, seasonal fluctuations as an urgent health crisis.

By convincing healthy adults that a slightly low lab reading was a toxic threat, they manufactured a massive market for expensive proprietary supplements and endless diagnostic tracking. It is a classic wellness marketing playbook: manufacture an epidemic of deficiency, and then sell the cure.

Why Medicare Stopped Footing the Bill

The financial burden of this media-driven overtesting became so severe that federal programs had to step in. Vitamin D screening exploded to become one of the most frequently ordered laboratory tests under Medicare Part B, costing taxpayers millions of dollars for routine screenings that yielded zero clinical benefits.

Because the vast majority of these tests are completely unnecessary, the Centers for Medicare & Medicaid Services (CMS) cracked down. Today, Medicare enforces strict Local Coverage Determinations (LCDs) that restrict reimbursement for routine screening. Medicare will now generally only cover the test if a patient has an explicit, documented medical necessity—such as chronic kidney disease, severe hypercalcemia, or advanced osteoporosis. If a clinic orders a routine screening test without a valid diagnostic reason, the test is deemed medically unnecessary.

Real Side Effects and Adverse Drug Reactions (ADRs)

Many people mistakenly assume that over-the-counter vitamins are completely harmless. This is a dangerous misconception. Because Vitamin D is fat-soluble, it accumulates directly in your body’s tissues instead of being safely flushed out in your urine like Vitamin C.

When people follow unmonitored advice from television or social media and take massive megadoses, they run a serious risk of Adverse Drug Reactions (ADRs):

Hypercalcemia: Excess Vitamin D forces the body to absorb too much calcium, leading to nausea, vomiting, muscle weakness, and profound confusion.

Severe Kidney Injury: A spike in blood calcium forces the kidneys to work under extreme stress, frequently causing painful kidney stones and potential long-term kidney damage.

Tissue Calcification: In severe cases of toxicity, calcium deposits can settle into your soft tissues, stiffening blood vessels and inducing life-threatening heart arrhythmias.

The Prescription: Stop the Pills, Just Eat Right

The scientific consensus has finally caught up to the hype. Current clinical guidelines from the Endocrine Society firmly advise against routine Vitamin D screening or high-dose supplementation for healthy adults under the age of 75.

The message from the medical community is loud and clear: stop overcomplicating your health with synthetic supplements and focus on eating a balanced diet.

Focus on whole foods: You can naturally get your required baseline of nutrients by consuming fortified milk, wild-caught fatty fish, eggs, and a variety of whole foods.

Get moderate sun exposure: A few minutes of daily sunshine provides your skin with all the natural synthesis it needs.

Listen to your primary doctor: Do not let a celebrity on a screen dictate your medical care. If you genuinely have an underlying condition that impacts your bone density, your local provider will evaluate you safely.

Before you request another automated lab panel or buy an unverified supplement online, talk to a real medical professional who looks at your complete clinical history.

Between managing a career, navigating the rising costs of family life, and keeping up with the daily pressures of modern parenting, stress has evolved from an occasional nuisance into a permanent fixture of daily life.

In our practice at East County Internal Medicine, we are seeing an unprecedented surge in patients battling severe stress and anxiety. It is spreading rapidly across Manatee County, placing an especially heavy burden on younger parents who are constantly juggling competing pressures.

Too often, people try to “tough it out.” But as internists, we see the hidden reality: anxiety is not just a mental battle—it is a physical disease that degrades your body over time.

The Dangerous Ripple Effect of Chronic Anxiety

When anxiety goes untreated, it rarely stays confined to your thoughts. It triggers a systemic domino effect that directly compromises your physical health:

Chronic Insomnia: Anxiety keeps your nervous system in a constant “fight-or-flight” mode. This floods your body with cortisol and adrenaline, making deep, restorative sleep nearly impossible.

Diminished Quality of Life: Constant worry drains your daily energy, eroding your patience, your relationships, and your ability to present at home.

Poor Work and School Performance: Anxiety impairs the prefrontal cortex of the brain, directly damaging your focus, memory, and decision-making capabilities.

A Gateway to Chronic Disease: Prolonged, unmanaged stress creates chronic inflammation, spikes your blood pressure, and strains your cardiovascular system. Over time, this significantly increases your long-term risk for heart disease, type 2 diabetes, and gastrointestinal disorders.

As strange as it sounds, we as humans are actually addicted to our anxiety. It’s all fun and games until it crosses the line. Once it interferes with your sleep, or work, schooling or relationships, it’s become pathological – Shaan Kunwar DO

Don’t Suffer in Silence: Rethinking Medication

The most heartbreaking trend we see is patients suffering in silence for months—or even years—out of fear or stigma surrounding mental health treatment.

Many patients express hesitancy about prescription medications. It is completely natural to want to understand what you are putting into your body. However, medical intervention must always be viewed through the lens of a clinical risk-to-benefit analysis.

While medications are not always the automatic first choice for mild stress, they become a vital, evidence-based necessity when the symptoms of anxiety begin to outweigh the potential risks of treatment. When anxiety is actively destroying your sleep, your health, and your ability to function, the medical risk of leaving that disease untreated is far greater than the risk of a carefully managed prescription.

Partner with an Established Expert

You do not have to navigate this overwhelming cycle alone, and you do not have to rely on trial-and-error to get your life back.

As an established expert in internal medicine, Dr. Kunwar specializes in diagnosing and treating the complex intersections of mental stress and physical illness. Our practice focuses on comprehensive care—evaluating your lifestyle, your sleep patterns, and your physical health to create a safe, balanced treatment plan tailored exactly to you.

Anxiety thrives in isolation, but it is highly treatable. Let’s protect your health, your family, and your quality of life before stress transforms into a chronic medical condition.

When you look at your monthly medical expenses or watch health insurance costs skyrocket, it is easy to wonder where all that money is actually going.

The disturbing reality? A massive portion of it is leaking right out of the healthcare system.

According to recent data from the Federal Trade Commission (FTC) and national senior advocacy networks, Medicare loses an estimated $60 billion to $90 billion every single year to systemic exploitation. To put that into perspective, the Centers for Medicare & Medicaid Services (CMS) recently launched an unprecedented crackdown, saving a record-shattering $41.9 billion in a single fiscal year by aggressively freezing suspicious payments.

But how did the problem get this massive? Who is behind it? To truly understand the problem, you have to look past the headlines and understand the distinct differences between Fraud, Waste, and Abuse (FWA)—and how the entire corporate healthcare structure has turned “working the system” into a highly coordinated campaign.

The Three Villains: Fraud vs. Waste vs. Abuse

While the terms are often lumped together under the “FWA” umbrella, CMS clearly distinguishes among the three based on two major factors: intent and medical necessity.

1. Fraud: Intentional Deception (The Criminals)

Fraud requires malice, deliberate intent, and knowledge that the action is wrong. It is a criminal act designed to steal money directly from taxpayers and the Medicare Trust Fund.

Examples: Billing for a patient who is deceased, charging for surgeries or lab tests that were never performed, or falsifying diagnoses to authorize expensive equipment.

The Players: Syndicate crime rings, fraudulent medical equipment suppliers, and rogue telemarketing companies that trick seniors into giving away their Medicare numbers.

2. Waste: The Misuse of Resources (The Inefficient Bureaucracy)

Waste does not necessarily involve a crime or criminal intent. Instead, it is the careless overutilization or misuse of healthcare resources driven by a convoluted, bureaucratic system.

Examples: Ordering a repetitive round of bloodwork just because a hospital’s electronic records software didn’t communicate with a clinic across town, or over-prescribing more medication than a patient actually needs.

The Players: Disconnected health systems, poorly integrated electronic records systems, and administrative bottlenecks that force duplicative care. [2]

3. Abuse: Violating Sound Medical Practice (The System “Hackers”)

Abuse happens when providers or corporate entities engage in practices that are inconsistent with sound professional standards or fiscal business practices. It results in completely unnecessary costs to the Medicare program.

Examples: “Upcoding” (billing a simple 15-minute checkup as a complex, 60-minute emergency visit to collect a higher fee) or ordering excessive diagnostic imaging to maximize profits.

The Players: Corporate-owned medical groups, hospital billing departments under pressure to hit revenue targets, and large pharmaceutical manufacturers exploiting regulatory loopholes. [4]

Who Are the Real Players?

When most people think of healthcare fraud, they picture an individual bad actor or a shady scammer calling seniors from an overseas boiler room. While those scams are very real, they represent only a fraction of the issue.

The largest contributors to the Medicare drain are institutional players and systemic campaigns.

Corporate Platforms: Federal prosecutors recently dismantled an online medical template platform that facilitated over $1 billion in fraudulent Medicare claims by generating fake doctor orders for unnecessary medical equipment in exchange for corporate kickbacks.

The Insurance Game: Under corporate insurance frameworks like Medicare Advantage, billions of dollars are routinely flagged as “improper payments” due to inaccurate risk-adjustments—where insurance companies artificially make patients appear sicker on paper to extract higher capitation payments from the government.

The Fee-for-Service Loophole: Traditional health systems operate like an assembly line. Because they are paid based on the volume of tests and procedures they bill rather than the quality of care they provide, the corporate structure itself incentivizes medical abuse and waste. [5]

The Consequence: You Pay the Price

Every dollar lost to fraud, waste, and abuse chips away at the fiscal stability of public healthcare programs. To compensate for these massive losses, insurance companies hike up commercial premiums for individuals under 65, hospitals raise their baseline prices, and government regulations become more burdensome for the honest, independent doctors trying to take care of you.

Ultimately, the patient ends up trapped in a system designed to maximize billing codes rather than health outcomes.

The Intelligent Alternative: Direct, Self-Pay Primary Care

At East County Internal Medicine, we choose to opt out of this broken cycle entirely. By operating as a transparent, cash-and-self-pay primary care practice right here in Lakewood Ranch, we have eliminated the third-party insurance middlemen, the complex billing departments, and the corporate pressure to cycle through dozens of unnecessary codes.

We keep healthcare simple, honest, and completely focused on you:

No Hidden Fees or Upcoding: Our pricing is entirely transparent—$175 for new patient visits and $125 for follow-ups. You will never receive a surprise bill weeks later because a code was manipulated.

Uncompromised Medical Decisions: Dr. Shaan Kunwar works directly for you, not an insurance company or a corporate board. Your treatments, tests, and referrals are based strictly on what you actually need—eliminating corporate waste and abuse.

Unhurried, Accessible Care: By cutting out the administrative waste, we can offer unhurried visits and same-day or next-day availability when you are sick. [6, 7]

You don’t have to be a casualty of a wasteful, multi-billion dollar corporate healthcare campaign. Experience the peace of mind that comes with clear, honest, direct medical care. [8, 9]

Medical residency is notoriously brutal. For years, young doctors push through 80-hour workweeks, minimal pay, and immense emotional strain. They tell themselves that once they finally graduate, they will find autonomy, balance, and a rewarding career.

Then comes graduation. New physicians are handed a stack of paper—a thick, complex, jargon-heavy employment contract. Eager to finally start practicing and facing an average of $250,000 in student debt, many sign on the dotted line without a second thought.

This is where the rubber meets the road. This exact moment is the true crux of the physician burnout crisis.

The Reality: A Retention Crisis by the Numbers

According to a shocking joint report by Jackson Physician Search and the Medical Group Management Association (MGMA), the transition from resident to attending is broken:

60% of new physicians leave their very first job within the first 1 to 3 years.

1 in 4 residents actively consider walking away from their new employer within just 12 months.

69% of hospital administrators naively expect new hires to stay for 6+ years, revealing a massive disconnect between leadership and frontline doctors.

When 60% of highly trained professionals abandon their first workplace almost immediately, it is no longer an individual issue. It is a systemic crisis.

Why Your First Contract is Driving Physician Burnout

Burnout does not just stem from long shifts or demanding patient panels. True burnout is a reaction to a total loss of autonomy, unfair administrative burdens, and deceptive workplace politics. The root of these issues is almost always buried inside that initial, unnegotiated contract.

The Control Illusion: New doctors believe they are signing up to heal patients. Instead, they find themselves trapped by corporate governance, inflexible schedule metrics, and zero say in administrative policies.

The “Standard Contract” Trap: Hospital recruiters frequently claim a contract is “standard” and non-negotiable. Young physicians accept this at face value, completely unaware that they can—and should—push back.

The Emotional Toll: Leaving a job after just 24 months creates massive personal and professional upheaval. It breaks continuity of care for patients and leaves the physician feeling like they have failed, when in reality, the contract failed them.

The Solution: We Must Mandate Job Market Training

Medical schools and residency programs do an incredible job teaching clinical medicine. However, they do a catastrophic job teaching the business side of medicine. Residents graduate with elite surgical or diagnostic skills, but possess zero training in how the modern job market actually works.

To solve the burnout epidemic, we must change how we prepare early-career physicians:

Incorporate Business Curriculum: Residency programs must include formal modules on contract literacy, RVU compensation models, and healthcare economics.

Normalize Professional Advocacy: Young physicians need to know that negotiating for better boundaries, support staff, and administrative time is not “greedy”—it is necessary for self-preservation.

Utilize Expert Guidance: Doctors must be encouraged to use legal review teams or specialized services like Resolve to level the playing field against massive corporate healthcare systems.

A Message to Our Future Peers

At East County Internal Medicine, we believe that protecting the health of our community starts with protecting the health of our physicians. Your first job contract is a major, life-changing decision. Treat it with the same rigorous care you give to a critically ill patient.

Do not just sign it. Understand it, negotiate it, and advocate for yourself—because a burned-out doctor cannot heal anyone.

If you feel like your doctor’s office has turned into a giant corporate assembly line lately, you are not imagining…

If you feel like your doctor’s office has turned into a giant corporate assembly line lately, you are not imagining it.

Across Florida, a massive wave of consolidation is happening behind the scenes. Large hospital systems, insurance conglomerates, and private equity firms are aggressively buying up local, independent family practices.

The result? The traditional, independent neighborhood doctor is becoming a rarity. And unfortunately, it is the patients who are paying the price.

What Corporate Consolidation Means for You

When a local practice gets absorbed by a massive healthcare network, the entire culture of your care changes. You might notice:

•	The “7-Minute” Visit: Corporate medicine operates on volume. Doctors are pressured to see as many patients as possible, forcing them to rush through appointments.
•	Worse Access, More Gatekeepers: Trying to get an appointment or ask a quick question means dealing with automated phone trees, complex online portals, and tiers of administrative staff.
•	Hidden Fees and Billing Surprises: Corporate health systems often add “facility fees” or complex billing codes to your bill, leaving you with confusing, unexpected costs weeks after your visit.
•	A Lack of Personal Connection: You become a medical record number on a spreadsheet rather than a person.

Breaking the Mold: True Independent Medicine

At East County Internal Medicine, we watched this corporate trend taking over Lakewood Ranch and decided to do something completely different.

When we opened our doors, we chose to be 100% independent. We don’t answer to corporate boards, we don’t let insurance companies dictate your treatment, and we don’t charge hefty, ongoing concierge retainer fees just to keep you on a list. Instead, we operate on a straightforward, cash-on-demand model.

The East County Internal Medicine Difference

By cutting out the corporate middlemen and the insurance red tape, we can bring medicine back to what it should be: a direct relationship between you and your doctor.

•	Transparent Pricing: You know exactly what your visit costs upfront. No surprise bills in the mail, no hidden fees, and no insurance deductible headaches.
•	Time with Your Doctor: When you book a visit with Dr. Shaan Kunwar, DO, you get his undivided attention. We don’t rush you out the door to hit a corporate quota.
•	Care When You Need It: Our model allows us to maintain a highly stable, personalized practice.

Most patients view primary care as a simple commodity. They assume that a doctor’s visit is a standardized product—you book an appointment, get your vitals taken, spend a few minutes with a provider, and leave with a prescription. Because the healthcare industry often markets itself this way, it is easy to assume that all medical practices deliver the exact same level of care.

However, there is a hidden reality that most patients are completely unaware of: primary care is actually delivered across wildly varying tiers of quality. Understanding these tiers is essential to managing your long-term health and avoiding the pitfalls of the modern corporate medical machine.

Tier 1: The Corporate Assembly Line

The most common tier of primary care today is the corporate or hospital-owned clinic network. In this model, healthcare is heavily commoditized.
•	The Dynamic: Large healthcare conglomerates treat medical offices like factories, measuring success by the volume of patients shuffled through the door.
•	The Reality: Doctors are often constrained by rigid corporate metrics, forcing them to limit patient encounters to 10 or 15 minutes.
•	The Outcome: Patients rarely see the same physician twice, dealing instead with a rotating cast of practitioners. This erodes continuity of care, as nobody truly knows your comprehensive medical history.

Tier 2: The Purely Virtual Intermediaries

With the rise of telehealth, a secondary tier of corporate care has emerged: digital-only healthcare platforms.
•	The Dynamic: These platforms promise ultimate convenience, letting you book a doctor on your smartphone in minutes.
•	The Reality: While highly accessible, virtual-only care decouples medicine from the local community.
•	The Outcome: If you develop an acute issue requiring a physical exam, a diagnostic test, or an urgent referral to a local specialist, these national virtual groups often fall short. They lack a physical footprint and deep roots in your regional medical community.

Tier 3: The Independent, Patient-Centered Standard

The highest tier of primary care breaks away from corporate mandates entirely. This is independent medicine, where care is tailored entirely to the individual rather than a corporate bottom line.
•	The Dynamic: Decisions are made exclusively between the patient and a dedicated physician who manages your care directly.
•	The Reality: Without corporate administrators dictating appointment times or forcing unnecessary referrals, independent practices prioritize thorough, unhurried consultations.
•	The Outcome: Patients experience genuine continuity of care, leading to highly personalized preventative strategies, more accurate diagnoses, and stronger physician-patient relationships.

The East County Internal Medicine Difference

At East County Internal Medicine (ECIM), we built our practice to completely bypass the corporate assembly line. Founded in 2017 as an independent, physician-owned practice in Lakewood Ranch, we treat primary care as an essential partnership, not a commodity.

Led by Dr. Shaan Kunwar, DO, our small, dedicated team bridges the gap between local accessibility and modern flexibility. We combine a permanent physical office in East Manatee County for in-person visits with statewide, secure telehealth services across Florida.

Our independent model allows us to deliver elite, Tier-3 primary care at a reasonable fee without resorting to expensive concierge models:
•	Guaranteed Continuity: You will always see the exact same solo physician—Dr. Kunwar—whether you walk into our clinic or connect with us online.
•	Real Accessibility: We do away with corporate call centers. We feature live telephone answering, same-week appointment availability, and rapid responsiveness when you need us most.
•	True Autonomy: Because we are entirely independent, our medical guidance is based strictly on clinical data and your unique needs—never on corporate quotas or hospital network mandates.
Healthcare should never be treated like a one-size-fits-all commodity. Discover the difference that independent, dedicated internal medicine can make for your health.
________________________________________
Ready to experience primary care tailored to you?
